I am looking for some tips on how other institutions evaluate the effectiveness of their field placement sites/agencies and field supervisors. Do you use any particular assessment tools, surveys or incorporate student feedback? Examples are welcome!

Hi Morgan,

We ask each student to complete an evaluation of their field placement/supervisory process at the end of their time in the program.  I am attaching the evaluation we use.  Hope this helps!
