We don't have a questionnaire, but in Seminar they have a weekly reflection assignment that I've adapted from the textbook by Larkin, S. A Field Guide for Social Workers.
And we also have the students complete the same evaluation/assessment that their supervisors do at the end of each semester in practicum.
For academic classes I have given weekly journals as assignments in the past and have found that helpful. It's a lot of work for the teacher.
I've heard from another professor in sociology that she does "exit" polls with her students through our online learning platform.
